Unit Summary

The Community and Population Health Services (CPHS) portfolio supports the advancement of integrated health and social services from hospital to community. The Program is responsible for the development and advancement of innovative partnerships with peer, community, academic partners, and provides expert leadership in the area of pre- and post-hospital transitions.

CPHS is accountable for HHS' Population Health, Bundled Care and Alternate Level of Care strategies. The portfolio provides system-wide support through the Site Administrative Managers, Managers of Transitions, Transitional Care & Discharge Specialists, ALC Navigators, Centralized Care and Transition Team, Hospital to Home (H2H), Integrated Comprehensive Care (ICC) Program, Medical Psychiatry Consultation Liaison Team, Substance Use Services and in partnership with HNHB Home & Community Care Support Services.

Position Summary

The Nurse Practitioner (NP) in the Community & Population Health Program will be registered with the College of Nurses of Ontario in the Extended Class - Adult and work within the scope of practice of the RN-EC

As a member of the Centralized Care and Transition Team (CCaTT) , the NP will work within an interdisciplinary team, the Emergency Department team and other team members utilizing a patient and family centered model of care. Additionally, the CCaTT NP will be involved in the Virtual Nursing Care Program supporting COPD and HF patients in accordance with the telepractice standards of the College of Nurses of Ontario, and HHS standards, policies and procedures. CCaTT NPs are responsible for providing direct patient care including : assessment and treatment of patients with multi-morbidity often impacted by social determinants of health and frailty; development of integrated care plans supporting seamless transition from hospital to community, and post-discharge follow-up for a sub-set of patients including those enrolled in the Virtual Nursing Care Program.

As an NP at Hamilton Health Sciences, you will tutor and mentor nursing and other health professionals as well as students as appropriate; participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate, and / or lead programs, presentations, publications, evaluations and other system changes to continue the enhancement of interdisciplinary care teams. The CCaTT NP will be instrumental in working with program leadership and stakeholders to develop and execute strategic plans, and to advance best practices for operational excellence and sustainability.

Schedule Work Hours

Monday to Friday with some weekends and holidays

Qualifications

1. Current Certificate of Registration in the College of Nurses of Ontario Extended Class - Adult

2. Completion of a Masters Degree in Nursing

3. Minimum of 5 years recent (within 2 years) clinical experience working with older adults presenting to hospital with multi-morbidity, frailty and social care needs

4. Demonstrated knowledge of the chronic disease management framework

5. Demonstrated knowledge of community resources to support integrated discharge planning with community partners

6. Demonstrated ability to critically appraise the literature, evaluate and implement best practice protocols

7. Demonstrated knowledge in quality improvement and the use of quality improvement methods and tools

8. Proven leadership abilities, including the ability to think strategically, influence decision making, affect culture and behavior

9. Proven effective communication and presentation skills

10. Proven success in implementing innovative, effective, value-added initiatives using change management and project management theory and frameworks

11. Demonstrated ability for independent action, initiative, problem solving, organization and prioritization

12. Demonstrated critical thinking, interpersonal and communication skills, both written and verbal

13. Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations

14. Demonstrated ability to utilize information technology
