About HART Hub
HART Hub provides low-barrier, client-centred services to individuals experiencing homelessness and housing instability. Our approach is grounded in dignity, respect, trauma-informed practice, choice, and practical support.

As part of our expanding stabilization services, HART Hub will operate 13 stabilization units providing 24/7 accommodation and support for individuals who require a safe, stable environment while working toward longer-term housing and community stability.

We are seeking compassionate, reliable and skilled Case Workers to join our 24/7 stabilization team.

They develop trusting relationships with residents, support day-to-day needs, identify emerging concerns, assist with service connections and contribute to individualized housing and stabilization plans.

This position is well suited to someone who understands that stability begins with relationship, safety, dignity and consistency.

The Case Worker does not work in isolation. They collaborate closely with the HART Hub Case Management, Housing, Primary Care, Addictions, System Navigation and leadership teams to ensure residents receive coordinated support.

· Encourage skills that support successful transition to permanent housing.

· Participate in discharge and transition planning.

24/7 Stabilization Operations

· Maintain a safe, welcoming and respectful environment within the stabilization program.

· Complete scheduled wellness checks and resident contacts in accordance with HART Hub procedures.

· Respond appropriately to resident concerns, incidents and urgent situations.

· Follow established emergency, safety, incident reporting and escalation procedures.

· Monitor the overall environment of the stabilization units and communicate emerging concerns to the appropriate supervisor.

· Assist with maintaining clean, organized and welcoming common areas.

· Maintain accurate, timely and objective client documentation in HIFIS and PSS.

· Complete case notes, incident reports, shift documentation and other required records.

· Document significant changes, concerns, interventions and follow-up requirements.

· Maintain confidentiality and protect resident privacy.

· Communicate effectively with colleagues and partner service providers within appropriate privacy and consent requirements.

· Participate in team meetings, case conferences and other collaborative planning processes.

· Work collaboratively with Case Managers, Housing Coordinators, System Navigators, Primary Care, Addictions staff, Program Coordinators and leadership.

· Participate in multidisciplinary discussions regarding resident needs and service planning.

· Provide relevant frontline observations to support coordinated case planning.

· Contribute to a positive, respectful and supportive team culture.

· Participate in training, supervision and professional development.
