Overview
We are hiring a Temp Full Time Social Worker, Toxic Drug Response and Priority Populations to join our team in the Hope / Agassiz / Harrison Hot Springs / Kent, B.C. areas. This position reports administratively to the Manager, Clinical Operations and clinically to the Clinical Practice Leader and involves working within an interdisciplinary team to provide comprehensive services and brief therapeutic clinical counseling to clients and their families.

Responsibilities
- Conducts comprehensive social work and psychosocial assessments by interviewing the client and / or their family, obtaining relevant information, gathering social data and interpreting the data using tools in accordance with professional standards, clinical policies, legislation and practices.
- Provides comprehensive therapeutic services by exploring psychosocial responses to identify problems, personal concerns, treatment objectives and facilitates change in coping style, behavior, attitude and feelings; coordinates services with clients, families, community supports and the interdisciplinary team; manages cases and facilitates meetings.
- Collaborates with the interdisciplinary team in client care planning; follows up and provides ongoing monitoring of complex psychosocial situations; evaluates client’s response to interventions.
- Supports a seamless approach to care and service delivery across the healthcare system by participating in admission and discharge processes; coordinates comprehensive care plans and community services; collaborates with hospital and community services to utilize resources effectively.
- Partners with community services, health and social agencies, including provincial Ministries and physicians, in assessment and management of individuals at risk, making referrals to appropriate resources as needed.
- Facilitates timely exchange of information regarding client care between acute care and community services to support individualized discharge planning; works with providers to ensure client care needs are met.
- Acts as a clinical resource by providing information about community resources, facilitating referrals, and ensuring client capacity decisions are respected in collaboration with the interdisciplinary team.
- Maintains patient records by documenting assessments, charting information and preparing progress notes and reports in accordance with standards, policies and regulatory requirements.
- Maintains workload statistics and contributes to reporting to support program planning and improvement.
- Develops, coordinates and delivers in-services, seminars, psycho-educational sessions and training with the interdisciplinary team.
- Participates in local committees to identify opportunities for change and to enhance patient care services and reduce gaps.
- Initiates and carries out research activities and program planning relevant to patient, family, hospital and community needs as assigned.
- Arranges and participates in home / community visits with other healthcare professionals and provides guidance on available community resources.
- Performs other related duties as assigned.
Qualifications
Education, Training, and Experience :
Current full registration with the BC College of Social Workers. Bachelor of Social Work from an accredited School of Social Work. Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training and experience.
Valid BC Driver's License and access to a personal vehicle for business-related purposes.
