Reporting to the Director of Care the RAI / MDS Coordinator plays a pivotal role in enriching the lives of residents by coordinating assessments of resident’s care needs and evaluates the quality and effectiveness of that care through analysis of quality indicators, outcome scales and benchmarking. Drives the resource utilization and funding.
This is a Full Time Days role, though extended hours may be required from time to time. The environment is fast-paced and deadline-driven, requiring high attention to detail and strong time management skills. The role includes regular use of computer systems and collaboration with cross-functional teams.
Key Responsibilities :
- Coordinates the completion of LTCF / RAI-MDS assessments within established timelines, including associated assessments and care plans
- Establishes an effective auditing process to ensure accurate data.
- Coordinates education for new staff in relation to LTCF / RAI-MDS and POC charting.
