Status : Temporary Fulltime 6 months ending July 2025

- Completes assessment and / or screening to identify social issues.
- Identifies and implements plans to address social issues
- Link and collaborate with a wide range of community / nonprofit and government service agencies programs and research additional resources to facilitate the implementation of patients care plans and discharge. Service areas may include but are not limited to : housing income supports identification replacement community supports employment and educational activities and services and linkage to case management services.
- Accompany patients on community outings as deemed necessary to facilitate transition to the community.
- Provides practical help and supports advocacy coordination individualized support problem solving direct assistance training and supervision to help clients obtain the necessities of daily living (i.e. medical dental legal and advocacy services financial support supportive housing and money management).
- Provides supportive counselling to patients and their families.
- Collaborates with the interprofessional team to promote evaluate and deliver services to patients / families and significant others / substitute decision makers / powers of attorney based on the needs of the program.
- Attendance and participation in various team and patient / family meetings in order to provide updates on patients current status and care planning goals as needed and as it relates to CSW activities.
- Collaborates with community / outpatient service providers.
- Provides updates on resource and community information to help inform team decisions regarding discharge planning.
- Participate in committees taskforce groups program process improvement initiatives and discipline specific subcommittees as required.
- Researches collaborates with and provides education on resources relating to available services within private nonprofit and / or public sectors according to patients needs.
- Maintains awareness and knowledge of community agencies and resources.
- Develops and maintains contact with appropriate community agencies and other resources that support specific patient population.
- Obtains provides and exchanges relevant information to referral sources.
- Collaborates with team members to refer patients to the appropriate community organizations and / or supportive services as required.
- Provide education to patients / families and significant others / substitute decision makers / powers of attorney etc. related to applicable community services and how to access them. Such service areas may include housing income supports community supports work / educational activities and services and linkage to other community health service(s). Coordinates and collaborates patient involvement with appropriate health care worker(s) community agencies and other resources.
- Provide education to patients families staff and community around community resources and services as needed.
- Participates in ongoing professional development
- Upon receipt of request for consult conducts CSW assessments and determines / implements appropriate treatment goals that meet the needs of the patient / family / external support.
- Using the health information system documents receipt of referral assessment ongoing interventions and discharge plan.
- Reviews patient health record to facilitate intervention.
- Records and compiles daily workload measurement statistics as required by the hospital
