Case Management Clinician, Urgent Needs / Brief Support Direct Service - Regulated - (OPSEU) Full-Time, Contract (Approximately 12 months) Multi-Site Accountability; Oxford Catchment Area (Oxford Zone) - Application Deadline : March 23, 2025 -   An Urgent Case Management Clinician works within a multi-disciplinary team framework throughout Oxford County, providing rapid response and support to individuals presenting with urgent, complex barriers and needs, including those with a serious and persistent Mental Illness, Developmental Disorder, Acquired Brain Injury, and / or Concurrent Disorders.

The Urgent Needs Clinician will work closely with the CMHA crisis team and with community partners on the Oxford County Situation Table.

This program offers support for a time limited duration, and is brief and transitional in nature, focused on timely intervention and goal oriented engagement.  Consider revitalizing your career and making a difference in your community by joining our team!

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months.

In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.

WHAT WE'RE OFFERING : What you'll be making - Starting rate for this role is  $29.57  per hour (Level 1 of the Direct Service, Regulated Professional Grid) ; Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $36.71 per hour When you'll work  -  70 hours over a two-week period, Monday-Friday 8 : 30am-4 : 30pm What your benefits will be - you will receive 9% in lieu of health benefits Your pension plan  – Eligible for immediate, optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP) HOW YOU WILL BE MAKING A DIFFERENCE : Adhere to the core values and policies of the organization Strong understanding of recovery principles and their application to people with mental illness and addiction Working knowledge of the bio-psychosocial model of care and best practices for co-occurring disorders Protect and promote the welfare, dignity, and individual rights of our clients at all times Participate in the bio-psycho-social approach to patient care providing support using Motivational Interviewing Employ safe work practices at all times Maintain a professional manner at all times in the performance of duties Working knowledge of community resources Establish a therapeutic working relationship with clients supported Maintaining trusting, respectful and collaborative working relationship Work from a client centered, trauma informed lens Urgent Needs (Assessment, Crisis Support, and Follow up) : Provide support services to a caseload of urgent needs clients Assess clients’ mental, physical and emotional functional abilities and needs and investigate presenting resources Engage in standardized screening and assessments Providing skill development, linkages, resources, information, counselling and advocacy Crisis prevention and intervention, and safety plan Provide psycho-education around meds and symptoms and addictions Collaboration with care providers and community initiatives Engages in community outreach programming, Mobile Health Outreach Bus, and Naloxone programming Attends the Oxford Situation table to support those in the community, who may be acutely elevated at risk Provide assistance with health-related appointments as per recovery plan and organization policy Deliver appropriate level of navigational support based on client capacity Actively work on reducing barriers and increased access to care, frequent support Monitor symptomology, medication, and activities of daily living to help improve individuals’ ability to function on a day to day basis and within the community Providing Basic / Essential Life Skills training to Clients if and as require Maximize client participation in programming and pro-social community activities; actively promote self-efficacy according to a Motivational Interviewing approach.

This will also include activities such as early intervention, relapse prevention, crisis intervention, follow-up and recovery maintenance Successfully work with and support individuals from a broad range of cultural backgrounds based on a sensitivity to and understanding of the unique challenges they may face Provide coverage for other team members as volunteered / directed WHAT YOU'LL NEED TO APPLY : Police Vulnerable Sector Check Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months.

We require the following qualifications : RPN, RN, or Bachelor's or Master's Degree in Occupational Therapy, Social Work, or Psychology or related field is required (primary proof of credentials will be required during the hiring process i.e.

original degree / diploma or official transcript) Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (as an RSW), the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario Current, clear Police Vulnerable Sector Check (PVSC) Ability to attend work regularly We prefer the following qualifications : Valid Driver’s License Access to a vehicle Experience in working in a community-based setting with an extensive knowledge of social assistance programs and both generic and specialized local social support networks is essential

  • Experience in crisis intervention / management and crisis resolution Demonstrated experience working from a Recovery Focused Practice Experience and detailed knowledge of serious mental illnesses usually gained by several years of working with this population Experience in the treatment of Addictions Experience in the homeless sector Knowledge of Services in Oxford County Extensive knowledge of pharmaceuticals used in the treatment of mental illness & experience in monitoring / administration of medication Extensive knowledge of the support / treatment services network in Oxford County and of regional services commonly accessed by the Program Knowledge of legal system Experience working with individuals with an ABI or Development Disability Treatment of Addictions  First Aid & CPR Bilingual both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada INTERNAL APPLICATIONS :   To be considered as an internal applicant, current staff members are reminded to apply using their @cmhatv.ca or @slwar.ca email address.
