Position : Registered Dietitian, Permanent part-time (8.5hrs weekly)

Southbridge, the owner and operator of several long-term care homes and retirement communities in Southwestern Ontario, is currently seeking a Registered Dietitian to become part of a dynamic food and Nutrition Department. You will ensure that our residents are provided with nutritious and delicious meals, that are prepared and served with love.

RESPONSIBILITIES :

• As a member of the Clinical Team, the Dietitian consults with and advises the Food Service Manager and Director of Care concerning nutrition and hydration program needs. The Dietitian is responsible for nutritional assessment, nutritional care planning, implementation, and follow-up for residents.
• Completes nutritional assessments for each resident using RAI MDS 2.0 as required.
• Determine an individualized resident’s diet and other nutritional interventions to meet his or her needs by involving the resident, family members and care team.
• Develop, implement and evaluate nutritional care plans that include identification of nutritional concerns, goals and interventions for all residents.
• Maintain excellent communication with the Food Service Manager regarding the changing needs of the residents.
• Review the two seasonal menus for nutritional adequacy and provides approval in writing.
• Educate residents, family members, and team members regarding nutrition and hydration recommendations.
• Act as a member of the Interdisciplinary Team, acts as a representative for Southbridge, through participation in regional and professional activities as appropriate.
• Participate in interdisciplinary committees related to resident care, facility operation and Quality indicators as required.
