POSITION : Clinical Nurse Specialist Chronic Care - Diabetes Management DEPARTMENT : Patient Care CONDITIONS : Full-time

  • Facilitates an interdisciplinary approach to chronic disease management and self-management support as per the Ontario Chronic Disease Management Framework.
  • Provides direct, expert, comprehensive care / nursing services, incorporating the Chronic Care Model to improve clinical outcomes for patients with diabetes, through health promotion and preventive care, empowering patients and families to use self-management methodologies to positively affect their health outcomes.
  • Demonstrates expert knowledge and synthesis of advanced nursing practice to perform comprehensive health assessments and clinical decision making.
  • Assesses complex clinical care issues, develops, implements, and evaluates innovative approaches and programs addressing these issues.
  • Participates in program planning and coordination of patient care to facilitate continuity of care for the diabetes inpatient population including : Policy development and review, implementation of evidence informed practices to enhance inpatient care, provide support and leadership in transitions of care discharge planning and facilitates post discharge access to diabetes education and supportive care in the community.
  • Coordinates outpatient diabetes care to facilitate continuity of care for the diabetes patient population, Type I, complex Type II and supports diabetes care in the Gestational Diabetes Clinic.
  • Provides education and support for the patient who uses Intensive Diabetes Management (MDI, CSII) including Pump Therapy.
  • Uses a variety of strategies including face to face encounter, telephone, email to undertake patient assessment, nursing interventions including provision of medication adjustment advice, evaluation of effectiveness of care and follow-up care planning for the diabetes patient population (Type I, complex Type II and Gestational).
  • Flexibility to take on other duties related to program needs as assigned.

Consultation

  • Initiates timely and appropriate consultation, referrals, and collaboration with other health care providers.
  • Works collaboratively with internal and external health care providers and teams to assess, plan, implement, and evaluate clinical interventions.
  • Provides recommendations or relevant treatment in response to consultation or incoming referrals.
  • Shares expertise, provides consultation, and advocates as required at the regional, provincial, national level in area of diabetes management.

Leadership

  • Provides leadership in program development, implementation, and evaluation of quality improvement initiatives.
  • Acts as a mentor and coach to nursing colleagues and others to advance clinical practice.
  • Model positive acceptance towards change and support others to adapt to change.
  • Participates in and supports nurses in continuous quality improvement initiatives evaluating the effectiveness of interventions / nursing services on patient outcomes.
  • Participates in and provides leadership on interprofessional committees related to development of policies, procedures, protocols, guidelines, clinical pathways, standards of care, medical directives, education, and research as they pertain to the diabetes patient population.
  • Participates in and provides leadership to support alignment with evidence-based practice / expert consensus.
  • Anticipates future issues / changes and provides leadership and vision in strategic program planning.

Education

  • Designs and delivers specialized education programs for patients, families, and health professionals.
  • Plans, implements, and evaluates needs-based continuing education programs for staff and clinical partners to promote quality patient care.

Research

  • Enhances the scientific base of clinical practice by utilizing and supporting research.
  • Contributes to the broader context of nursing knowledge through presentations and publications.
  • Evaluates clinical research and incorporates findings into clinical practice where appropriate.

REQUIREMENTS :

  • Masters Degree in Nursing (candidates with a masters degree in progress will be considered)
  • Bachelors of Science Nursing Degree
  • Minimum of five (5) years recent and relevant clinical nursing experience, including medicine, and / or chronic disease prevention and management experience
  • Minimum three (3) years recent and relevant diabetes management experience in the context of other chronic conditions
  • Registered with the College of Nurses of Ontario
  • Certified as a diabetes educator
  • Knowledge and experience with patient self-management support
  • Proven ability in independent and collaborative decision making
  • Strong communication skills oral, written, electronic
  • Ability to work with adult learners and colleagues in a variety of settings using a collaborative approach
  • Ability to respond to questions and requests in a timely manner
  • Demonstrated commitment to own continuing academic growth and development
  • Excellent analytical and problem solving skills ability to respond to shifting priorities and to multi-task
  • Strong organizational skills with demonstrated ability to meet deadlines / timelines
  • Demonstrated computer literacy

PREFERRED :

  • Member of the Registered Nurses Association of Ontario
  • Member of the Diabetes Educator Section of the Canadian Diabetes Association
  • Canadian Nurses Association (CNA) and / or related certifications
  • Experience with clinical documentation in an Electronic Health Record
  • Recent and relevant diabetes management experience in the context of diabetes and pregnancy
