Responsibilities

• Planning, organizing, directing and evaluating residents’ nutritional care in collaboration with the interdisciplinary team.
• Providing nutritional care including assessments, reassessments and developing appropriate nutrition interventions in consultation with the interdisciplinary team.
• Developing therapeutic menus for special diets not included in the therapeutic menu extensions.
• Identifying risks related to nutrition care, dietary services and hydration.
• Implementing interventions to mitigate and manage nutrition and hydration risks.
• Providing nutrition / dietary counseling or education for residents and / or their caregivers, family members or substitute decision makers to support optimum resident nutrition care.
• Performing other duties as assigned.
