Position Summary :
The primary role of the Nurse Practitioner (NP) is to provide high quality, safe, effective and evidence-based care in partnership with patients and families, in accordance with the College of Nurses of Ontario Nurse Practitioner Practice Standards and GGH’s mission, vision, values and policies. The NP uses advanced nursing knowledge clinical practice expertise and provides leadership to promote optimal patient health, program development, process improvements and the enhancement of professional standards. The NP optimizes the patient’s experience and care outcomes by :
- Being accountable for one’s decisions and professional practice;
- Establishing a therapeutic relationship with the patient and family;
- Collaborating with other members of the patient’s care team;
- Coordinating, communicating and documenting patient care activities;
- Providing high quality, evidenced based, patient and family-centred care.

Nurse Practitioner (Registered Nurse in the Extended Class) with certificate of registration with the College of Nurses of Ontario with license to prescribe controlled substancesMaster’s degree in Nursing or equivalent preferredCompletion of the Prescription and Management of Controlled Drugs and Substances CourseExperience in clinical program development and evaluationExperience in development and implementation of Medical Directives and Policies and ProceduresBCLS certificationNP Acute care with a demonstrated interest and expertise in Geriatrics3-5 years of recent experience in Comprehensive Geriatric AssessmentStrong assessment skills of the complex medically acute adult patientAdvanced knowledge and skill working with various types of dementia, delirium, mental health issues, addictions and other neurological conditions in older adults.Demonstrated specialized skills in geriatric medicine health assessment, care planning and patient outcomesAdvanced knowledge of psychotropic medications in symptom management, monitoring / evaluating effectiveness and side effectsExperience and advanced knowledge and skill in working with patients with responsive behaviours.Training in P.I.E.C.E.S., Montessori Methods in Dementia, Validation CommunicationGentle Persuasive Approach Coach CertificationExtensive knowledge of community partnerships and geriatric resources in Guelph and Wellington CountyAbility to function as a content expert in geriatric care and act as an expert resource to the inter-professional team.Demonstrated commitment to best practice, nursing standards and life-long learningExcellent interpersonal and communication skills, both verbal and writtenStrong organizational and time management skillsStrong critical thinking, problem solving and decision-making skillsDemonstrated excellent assessment skillsDemonstrated ability to work independently as well as collaboratively within a team in a fast-paced and ever-changing environmentDemonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physiciansStrong clinical judgment and diagnostic reasoning skills, with a demonstrated commitment to collaborative practiceDemonstrated leadership in committee work, education, quality improvement, policy & procedure development, and knowledge translation initiativesCome and join our team if you are looking for an exciting opportunity where you will be supported and empowered.
