Pharmacist, Pharmacy Department

Responsibilities include but are not limited to : Responsibilities include but are not limited to :

• Participates in relevant rounds and care conferences to ensure safe and appropriate prescribing decisions are made, patient outcomes are monitored and adverse drug events are prevented
• Identifies, prevents and resolves drug-related problems by conducting medication reviews, and developing and monitoring patient care plans. Communicates patient care plans to the interprofessional team, patient, family and / or Substitute Decision Maker
• Ensures the seamlessness of continuum of care by conducting medication reconciliation when patients are transferred to or discharged from Baycrest
• Ensures appropriate and accurate preparation and dispensing of medications to designated Hospital programs
• Participates in departmental medication management projects and Pharmacy clinical programs
• Provides education to Pharmacy students as per the requirements of the Faculties of Pharmacy (University of Toronto and University of Waterloo)
• Provides oversight and direction for Pharmacy Technician(s)
• Ensures best practices are implemented and maintained to enhance medication safety and patient safety
• Performs other duties as assigned
