Health Care Aide / Personal Support Worker

Job Summary
The Personal Support Worker is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client’s needs.
Responsibilities
- Assist with personal care and hygiene
- Prepare meals
- Perform light housekeeping
- Accompany clients on outings
- Handle shopping and errands
- Assist with medication reminders
- Assist with ambulation and transporting clients
- Provide companionship
- Follow care plans
- Document and chart care activities
