Job description

Site: Queens Estate 265 Queens Dr North York ON M6L 3C6
Department: Transitional Care Program
Reports to: Manager Program Services
Hours: 37.5 Hours/Week
Salary: $27.51 (start rate determined in accordance with collective agreement requirements and is non negotiable)
Shifts: Day Shifts (every other Saturday or Sunday)
Status: Permanent Full Time
Closing Date: December 11 2025

RESPONSIBILITIES:

  • Provide social educational and recreational programming to create a positive and engaging experience for clients
  • Plan and organize group activities with other team members based on clients cognitive and physical capabilities
  • Program preparation including purchase of program supplies producing of program flyers activity calendar and etc.
  • Maintaining a safe environment for program operation including activity area cleanliness equipment maintenance IPAC compliance etc.
  • Transporting and/or escorting program participants to and from programs with the support of personal support workers
  • Documents clients abilities and records participation
  • Evaluate the success of events and activities gather feedback from clients and make improvements for future activities
  • Other duties assigned as required.

Qualifications :

  • Post-secondary education in social and health services at least at a Community College Level with work experience and knowledge in elderly social service or equivalent.
  • Knowledge and training in GPA PIECES and Restorative Care an asset
  • At least 1 year of working experience in community setting
  • Sensitivity to recognize the aging and physical needs of elderly clients
  • Good organizational and time management skills
  • Ability to work independently and as part of a team
  • Excellent organizational communication and interpersonal skills.
  • Proficient computer skills in Microsoft Office
  • Current certification in CPR
  • A valid vulnerable sector police record check completed within one year
  • Good attendance record is required
  • Current UHN employees must have successfully completed their probationary period have a good employee record along with satisfactory attendance in accordance with UHNs attendance management program to be eligible for consideration.
