Job description

ABOUT THE ROLE The Sherwood Park-Strathcona County Primary Care Network (SPPCN) is seeking a passionate and collaborative Nurse Practitioner to join our team in a unique and rewarding 0.8 FTE permanent position based in Fort Saskatchewan.

This role offers an exceptional opportunity to practice to full scope while supporting patients across three innovative programs.

The main area of work for this role is with our Hospital Transitions Clinic for Unattached Patients, an NP-led clinic designed to support unattached patients following hospital discharge.

The clinic provides timely post hospital follow-up care for all ages and supports patients with their primary care needs while supporting the transition to a long-term primary care provider.

In addition, this role contributes to SPPCN's Women's Health services supporting rural populations and provides care through The Bridge Youth Wellness Hub.

The Bridge clinic supports youth with concerns related to sexual health, contraception, STI testing, wound care, and general health and wellness needs.

  • 0.8 FTE – Permanent Schedule: Monday – Thursday with an evening shift on Tuesdays (12:00-8:15PM) Eligible for Benefits 9% RRSP matching program Paid vacation and sick time Health & Wellness Spending Account Pay range, based on experience $62.74 - $72.00 Education support, including funding and paid time for professional development KEY RESPONSIBILITIES Include, but are not limited to the following: Provide comprehensive health assessments and initiate appropriate treatments, according to currently accepted evidence-based practice Assessment of patient conditions, needs and goals in collaboration with the patient.

Provide professionally autonomous decisions in managing patients Initiate, recommend and facilitate appropriate referrals to community resources and programs Provide relevant patient education to prevent and manage clinical conditions Participate in evaluation of the clinic and its services Undertake disease management, incorporating monitoring progress, response to treatment, and promote the preventive aspects of patient care Support the transition of patient care to longitudinal care provider Assist in the implementation of strategies for improving the quality and provision of care for the practice population Any other duties that may be reasonably required for the success of the PCN Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies EDUCATION & EXPERIENCE Master’s or equivalent in Nursing with a focus on advanced clinical practice Three (3) years or more of related experience including advanced skills in caring for patients with chronic, complex, and/or specialized needs Nurse Practitioner, currently enrolled with CRNA extended practice roster.

In accordance with the Health Professions Act the NP must clearly identify to the PCN any work restrictions to the NP’s license.

CRNA places three restrictions on a NPs license to work with one of three patient populations, adult, pediatric or family-all ages.

When a NP is hired to work for the PCN she/he will be hired with the knowledge of the above restrictions in population of practice.

If the PCN requires a change in the population of practice the appropriate education and experience will be provided to the NP in accordance with CRNA regulations and approval.

SKILLS & ABILITIES Strong, effective written and oral communication skills, high level of perceptiveness, Critical thinking skills Customer service orientation At least basic computer skills in word processing and email Knowledge of, and ability to use, electronic medical records Demonstrates critical thinking, strong written and verbal communication, interpersonal, decision-making, and customer service skills, with a high level of perceptiveness, initiative, and accountability.

Will perform comprehensive health assessments and initiate appropriate treatments, according to currently accepted evidence-based practice Provide expert clinical care in managing the healthcare needs of patients/clients. The NP works from an extensive, multi-faceted base, integrating clinical, research, education and leadership roles into the provision of care. The NP exercises critical, complex clinical decision-making skills and directs patient/client care to ensure continuity across the healthcare continuum.

The NP understands that their decisions/actions have serious implications on patient/client health outcomes if errors are made. The NP practices within the governance of their regulatory organization and acknowledges the understanding of those regulations.

Perform comprehensive assessment, diagnosis (including the ordering and interpretation of tests), intervention (including the prescribing of medications), management, referrals and evaluation of patient/client care across the continuum of health care services.

Ability to work effectively in a dynamic, team-based environment with other health professionals, demonstrating flexibility and the ability to balance competing priorities Professional Development Responsible for maintaining continuing competencies through ongoing professional development including participation in education programs, research and continuous quality activities Adhere to the Canadian Nurses Association (CAN) Code of Ethics for Registered Nurses and his/her employer’s directives, policies, protocols and procedures Provide restricted activities and care within the full scope f practice as defined under the Health Professions Act (HPA) Responsible and accountable for his/her nursing practice and for ensuring that his/her registration is current, valid and entered on the NP roster under the HPA PERSONAL ATTRIBUTES AND COMPETENCIES Ability to build a good working relationship with healthcare professionals, clients and other members of the community Ability to prioritize, manage time effectively and be flexible in a very active work environment Respect and understanding of confidentiality Thrives in a complex changing environment High levels of integrity and professional behavior OTHER OCCUPATIONAL REQUIREMENTS A current and clear Criminal Record Check, including a Vulnerable Sector Check, is required prior to start date A valid driver’s license and access to a vehicle for travel is required Current standard BLS Provider certificate DISCLAIMER This job description indicates representative responsibilities and specifications of the position only and should not be considered to necessarily represent all responsibilities and specifications of the position.
