Access Healthcare Services is a leading home and community healthcare provider serving the Ottawa and Champlain region. Care Manager (RN/RPN) Personal Support Services - Ottawa East
REPORTS TO:
Program Manager Personal Support Services
POSITION PROFILE
As a Care Manager with the Personal Support Program you will provide and promote high-quality client centered care to those assigned to your caseload. Within the defined scope of practice as an RN/RPN this includes but is not limited to completing client assessments PSW delegations and training evaluation and update of care plans and client education and health promotion activities. The position requires excellent customer service skills working in collaboration with the interdisciplinary team and monitoring care provided to our community clients to ensure the highest level of care at all times. The role requires adherence to the principles of quality documentation and maintenance of client health records at all times.
PAY RANGE
$28.00 - $32.00 per hour (commensurate with experience)
