Position Summary and Scope of Service

The Service Facilitator will take an active role in promoting service and developing creative strategies to work with families and systems. The Service Facilitator promotes a positive nurturing environment for the family and works toward strengthening support networks enhancing knowledge competencies skills and family stability using a family-centered approach within an ecological model of practice. The Service Facilitator is responsible for evaluating the effectiveness and efficiency of direct interventions to ensure positive observable and beneficial outcomes to families. The Service Facilitator will provide direct clinical and consultative services to clients families or family supports and is familiar with and promotes a customer service focus in the provision of services to clients and other professionals.

The Service Facilitator provides services with the Mission and Values of policies and procedures of Clinical Services external legislation and regulatory bodies and all relevant professional standards of practice.

The Service Facilitator promotes processes that contribute to the areas of self-determination and the development of skills and competencies of individuals and care providers. These areas pertain to the entirety of the intervention including assessments treatment and implementation of service. The Service Facilitator is responsible for evaluating effectiveness and efficiency of interventions to ensure outcomes that are positive and beneficial to clients.

The Service Facilitator is familiar with social policy issues trends best practices in the field needs gaps or opportunities that may improve the lives of persons with disabilities or which promote social justice equity or accessibility.

The Service Facilitator is familiar with and adheres to all policies related to risk management and those of workplace activity promotes and follows all safety health and respectful workplace practices.

The Service Facilitator is involved in a variety of services and has responsibilities including the provision of direct and indirect services to individuals and families facilitating groups participating in research or educational activities facilitating access to services and maintaining statistics. The Service Facilitator is involved in activities that are related to optimizing enhancement of quality of life of individuals families and the community.

Qualifications

Educational:

  • Must possess a Bachelor of Social Work (BSW) Degree from an accredited university.

Professional:

  • Must be eligible for and maintain registration with the Manitoba College of Social Workers including meeting all prescribed continuing education requirements of the profession.
  • A minimum of 2 years social work experience in the area of developmental disabilities is preferred.
  • Ability to travel including to rural communities in Manitoba may be required. Travel by plane may be necessary in addition to overnight stays in or around a respective community.
  • Ability to work flexible hours as needed.
  • Ability to work from home is required. Services are delivered through a combination of virtual and in-person meetings.

Personal:

  • Ability to provide education and training in public forums.
  • Must possess a valid Manitoba drivers license and have access to a vehicle.
  • Possess excellent written and verbal communication skills.
  • Knowledgeable of the concepts of Quality Improvement (QI) and ability to implement services consistent with these concepts in a complex environment.

Duties and Responsibilities

The Service Facilitator Social Worker (working within the Community Stabilization Service) ensures a successful short-term interdisciplinary consultative service to people with intellectual disabilities whose living arrangements are at risk of breakdown due to complex challenging behaviours or other reasons.

Using a strengths-based person- and family-centered approach the Service Facilitator promotes a positive environment for the individual and works collaboratively to support formal and informal networks and enhance knowledge competencies and skills. The Service Facilitator takes an active role in promoting services and developing creative strategies to work with individuals families external service providers and systems as required.

The Service Facilitator is responsible for the overall facilitation of CSS. Under the direction of Manager Social Work and Counselling the Service Facilitator reviews and manages the CSS waitlist; gathers information from referring agents; and assists in the completion of a multi-disciplinary assessment report. The Service Facilitator is responsible for scheduling and chairing interdisciplinary and other meetings; and otherwise ensuring that services are delivered in a timely and time limited fashion. The Service Facilitator is responsible for coordinating education and training delivered by CSS team members and works collaboratively with the care team to achieve positive outcomes that contribute to the persons dignity quality of life and self-determination.

Clinical:

  • Demonstrated knowledge and experience using a person-and-family-centered model of service provision.
  • Knowledge of the history of services to people with disabilities current trends philosophies and practices concerning their care education and lifestyles.
  • Extensive knowledge and experience working with children and families and experience working in the area of developmental disabilities/mental disabilities.
  • Knowledge and ability to function within the legislation pertinent to practice including CFS Act Adults Living with an Intellectual Disability Act and PHIA.
  • Expertise in conducting social/family assessments using an ecological framework.
  • Knowledge of psychosocial and environmental factors involved in chronic disability which may impact family functioning or adjustment and the ability to define appropriate intervention strategies.
  • Knowledge of child development family relationships and dynamics.
  • Knowledge of Manitoba Guidelines for Identification and Reporting Abuse (CFS Act / Adults Living with an Intellectual Disability Act).
  • Ability to prepare an intervention contract with individuals and families appropriately assist in goal definition engage in problem solving processes focus on client strengths restructure or access appropriate supports define timeframes and plan for transition.
  • Ability to form appropriate professional/trust relationship with clients.
  • Demonstrated counseling skills including supportive adjustment and crisis counseling.
  • Ability to identify high-risk situations and respond appropriately.
  • Working knowledge of service systems and ability to creatively access or develop informal and formal supports or resources.
  • Ability to document plans and interventions accurately and in a timely manner using standardized program protocol and formats.
  • Ability to communicate openly and appropriately using positive interpersonal communication skills.
  • Knowledge of the dynamics inherent in the client-worker relationship and skilled in preparing for transition and in determining the nature of support required upon termination.
  • Ability to develop and conduct specialized group interventions and to document benefits and outcomes.
  • Ability to organize chair and conduct meetings.
  • Ability to understand support and practice interdisciplinary teamwork to provide services in the clinical educational or living milieu of the person with a developmental disability.
  • Ability to maintain an active caseload providing assessment service plan development intervention planning/implementation and service outcome evaluation.
  • Ability to work within direct mediated and consultative models of practice.
  • Understand ones own clinical limitations and seek support and consultation for clinical activities so as to do no harm.
