Looking for an RN Case Manager position in Cambridge, Ontario? We have both contract and permanent openings with options for on-site, hybrid, or remote work. In this RN Case Manager role, you will be responsible for daily patient monitoring and managing care transitions. You'll work with medical teams and families, ensure accurate record-keeping, and facilitate necessary procedures. Your critical thinking skills will be essential as you determine the best actions based on client policies and needs.
