Job description

Who we are

Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been delivering mental health services to Durham Region for over 60 years. It provides a broad range of client‑centered services, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. The work is delivered by inter‑professional teams that address both clinical issues and social determinants of health within a recovery‑oriented framework.

Overview of the role

The Nurse Practitioner offers comprehensive primary care to individuals, families and the community with a strong emphasis on health promotion, patient education and disease prevention.

Assessment

Working within the scope of practice of the RN (EC) to perform comprehensive, symptom‑directed assessments; synthesizing data from multiple sources to diagnose health problems; identifying lifestyle, psychological, sociocultural, and environmental risk factors; coordinating care through referral or consultation with other health providers and agencies.

Treatment

Providing nursing care and treatment that may include pharmacological, complementary, or counseling interventions; prescribing scheduled drugs within the RN(EC) scope; collaborating with physicians and allied health to develop individualized care plans; monitoring chronic illness; and implementing strategies to promote independent, healthy lifestyles.

Organizational Role

Maintaining current knowledge of policy manuals; participating in organizational meetings; contributing to efficient functioning and goal attainment; basing practice on theoretical knowledge and best practices; organizing priorities and workflow; and following established administrative procedures.

Risk Management

Minimizing risks by adhering to CMHA Durham policies; identifying, assessing, evaluating, and controlling risks; ensuring the safety and well‑being of staff and clients.

Qualifications

  • Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate or equivalent in good standing
  • Bachelor of Science in Nursing
  • Master’s degree in nursing or a related health‑care field (required)
  • Current registration with the College of Nurses of Ontario as an RN (EC) (required)
  • Current membership with the RNAO and evidence of liability insurance (required)
  • Liability protection or liability insurance (required)
  • Valid Ontario driver’s licence
  • Proficient in electronic medical records
  • Sound knowledge of the Mental Health Act, Health Care Consent Act and related legislation
  • Knowledge of psychotropic medications, usage and potential side effects
  • Strong written and verbal communication skills; ability to present information and lead meetings
  • Intermediate proficiency in French (asset)
  • Proficient with internet and Microsoft Office (Word, Excel, PowerPoint, Outlook)
  • Experience in injections, phlebotomy, administrative support (asset)
  • Ability to manage medication checks ensuring accuracy
  • Ability to build positive relationships with clients, community partners and represent the agency professionally
  • Excellent assessment, organizational and problem‑solving skills
  • Ability to work independently and as part of a high‑performance multidisciplinary team
  • Flexibility to adapt to change and support varied clinical duties

Effort

Physical demands include sitting, standing, walking, performing daily tasks and occasional lifting of walkers or wheelchairs. Requires concentration, attentiveness and creativity; management of medication administration, documentation, and use of nursing equipment.

Working Conditions

Work in a medical office and may be required to work in residential homes, physician offices, or shelters. Exposure to clients in crisis, abusive or irate clients, deadlines, distractions, and hazards such as blood, bodily fluids, and cleaning chemicals.
