Purpose
This Request for Supplier Qualification (RFSQ) is issued by HealthPRO Procurement Services Inc. (HealthPRO Canada) in order to qualify products/services (collectively Products) and suppliers/distributors (collectively Suppliers) for inclusion in HealthPRO Canadas Qualification Supplier Roster (QSR). Suppliers selected for inclusion will enter into a Qualified Supplier Roster Agreement (Roster Agreement).
During the term of the Roster Agreement, HealthPRO Canada may invite the qualified suppliers to compete in invitational second-stage competitive processes (Roster Competitions, of which one method may be RFP) involving the provision of the Products. If and when HealthPRO Canada requires the Products, HealthPRO Canada will invite all eligible suppliers on the Qualified Supplier Roster to participate in a Roster Competition.
Suppliers responding to this RFSQ must be a single legal entity that, if selected, intends to be included in HealthPRO Canadas QSR.
Pricing and other financial information is not required for qualification and should not be included in submissions to this RFSQ.

Access to the HealthPRO Canada RFSQ will only be sent to the email address from which the request is received.
June 01, to May 31, plus, at HealthPRO Canadas option, one additional two year term (the Option Term).
This RFSQ is subject to the provisions of the Canadian Free Trade Agreement, Chapter Five Government Procurement and the Comprehensive Economic and Trade Agreement - Chapter Nineteen - Government Procurement.
All bidders will be required to agree to HealthPRO Canada Contract Terms and Conditions which will be provided at the Roster Competition stage.
