The Social Services Support Worker plays a key role in supporting clients’ social and practical needs while enhancing access to essential community services. Working collaboratively with Grand River Community Health Centre (GRCHC) staff and community partners, this role will oversee the Outreach program and assist clients in navigating services related to identification, health care, and various application processes.
The Social Services Support Worker provides non-judgmental, client centred guidance, supports the implementation of social programming, and ensures all activities align with GRCHC protocols and policies.
A core component of this role is increasing community awareness of and access to the GreenShield Essential Medicines Program . The successful candidate will lead coordinated outreach, education, and application support activities aimed at improving uptake among eligible clients.
