Classification / Salary : Social Program Officer (Growth to R24) / Position starts at $39.30 to $41.65 hourly

About the role :

As a Facilitator at CLBC, you will represent CLBC as the primary contact for individuals with developmental disabilities and their families. You will support and facilitate access to generic services, informal supports and CLBC-funded services. Facilitators assist individuals and families to connect to their community, to build on the strengths and assets of the individual, and help them develop achievable goals. In this role, you will assist the individual and their families in identifying the activities and resources required to reach their goals, by supporting the individual to explore their capacity and enhance their ability to make informed decisions and choices. The Facilitator role requires excellent written and verbal communication skills, the ability to actively listen, understand and articulate the needs and desires of individuals and their families, and mediation skills.

Key Responsibilities :

  • Provide information about available supports and services and expected levels of support to address the individual's disability-related needs
  • Provide information about informal community supports, generic services, CLBC services, peer and family support services available
  • Provide advice and guidance to eligible individuals and their families to assist them in decision-making
  • Assist individuals who are in crisis and develop an effective crisis management approach
  • Providing support with life transitions for individuals and their families including assisting with changes in existing supports
  • Create and implement individual support plans, which include informal and formal safeguards

What you wi ll bring :

  • Preference will be given to candidates with a Bachelor of Social Work or an equivalent degree
  • Experience working in the Community Living field with children, youth or adults with developmental disabilities and their families
  • An equivalent combination of relevant education / experience may be considered
  • We are also looking for the ability to :

  • Understand and apply person-centred planning, goal-based planning and community engagement processes;
  • Apply appropriate analytical, problem solving, and judgement to the development and implementation of plans
  • Work with individuals and families to identify required supports without imposing personal or professional bias
  • Identify community resources and participate in, develop and enhance partnerships and links with community stakeholders
  • Research, record and share information relating to generic services and informal community supports
  • Resolve conflict with a wide range of stakeholders
  • Contribute to the stewardship of finances and other resources
  • Effectively represent CLBC in the community exercising tact and diplomacy
  • Additional Info :

  • Hybrid work - 3 days a day week office after passing a 6-month probationary period
  • Possession of a class 5 driver's license and a clean driver's abstract are required
  • Successful applicants are subject to a Criminal Record Check
