This Licensed Practical Nurse (LPN) position provides Case Management to clients living in the Airdrie area. This position requires advanced critical thinking abilities and a wide-ranging interventions skill set & experience. In collaboration with the interdisciplinary team, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers / families. The LPN is responsible for their nursing practice as defined by the CLPNA. There is also an expectation to be a mobile worker. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related purposes is a requirement. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting up to 30 lbs., lowering, carrying, pushing and pulling).

• As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills.
• You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures.
• You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Location Details : As Per Location. FTE : 1.00. Temporary Terms : Temp F / T Benefits; Date Available : 16-FEB-2026; Temporary End Date : 10-JAN-2027. Hours per Shift : 7.75; Shifts per cycle : 60; Shift Pattern : Days, Weekends. Days Off : As Per Rotation. Salary : $27.58–$36.13 per hour.
