Responsibilities :

• Provide compassionate care focusing on patient-centered services, including disease management, medication reviews, adherence, health assessments, and vaccinations.
• Offer counseling and education tailored to patient needs, considering literacy, hearing, and privacy.
• Develop care plans, follow up, and document appropriately.
• Collaborate with colleagues and participate in professional programs to deliver excellent health and wellness services.
• Ensure seamless patient experience across locations through active engagement in Fusion.
• Prioritize prescription safety, supporting pharmacy technicians and assistants as needed.
• Stay informed about pharmacy services and industry trends, attending training as necessary.
• Comply with all regulatory and company policies, including safety and quality standards.
