As Durham Region’s largest community based rehabilitative therapy provider we have been providing community rehabilitation services for adults and children since 1997. We are currently seeking qualified Registered Dietitians to provide services in the community.
RD Responsibilities :
Provide ministry funded, community based Registered Dietitian services within the client’s place of residenceProvide Dietitian assessment (through interview, standardized and informal assessment techniques) in the areas ofDevelop and implement best practice and evidence-based intervention in collaboration with clients / caregiversProvide education to client / caregivers and link to community agencies / resources when appropriateComplete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and college standardsRequirements :
Registered Dietitian with the College of Dietitians of Ontario (required)Experience in community setting is preferred but will consider new graduatesPediatric experience and / or a willingness to learn how to address the needs of the pediatric populationAbility to work independently and as part of an inter-professional teamExperience in assessment and counsellingKnowledge of chronic self-management principlesAbility to prioritize, manage self-scheduling and meet reporting deadlinesExcellent oral, written skills and report writingProficiency with computersStrong interpersonal and organizational skillsA willingness to cover Courtice, Bowmanville, Clarington, Newcastle, NewtonvilleAn ability to help with overflow in the other areas of Durham Region as requiredReliable transportation is required for this positionInclusion
