Job Description

The Program Manager, Integrated Care cultivates and manages high‑performing teams of Nurses, Personal Support Workers (PSWs), and Allied Health professionals. The role fosters a collaborative, innovative, and learning‑focused environment aligned with Spectrum’s Mission, Vision, and Values. You will ensure seamless program operations, leveraging data‑driven insights to optimize service delivery, enhance quality, and achieve key outcomes. You will also collaborate with external health system stakeholders to support the design and implementation of innovative programs, ensuring seamless care transitions.

• Lead program development and design clinical workflows for innovative care models while managing key internal and external stakeholder relationships.
• Recruit, onboard, and support team members, ensuring training, skill development, and the cultivation of best‑practice champions.
• Mentor the team to consistently meet high care quality standards.
• Collaborate with operational leadership to implement and maintain exemplary practices.
• Address quality, safety, and risk issues, developing improvement plans as needed.
• Monitor and report key performance indicators, initiating improvements or process changes when required.
• Contribute to improved client outcomes through evidence‑informed practices and resource efficiency.
• Integrate client satisfaction plans, addressing concerns promptly.
