About the position :

Working 7.5-hour shifts (Overtime may be required). S / he will provide in-home support such as basic infant care, infant stimulation, fostering healthy child development, self-care and coping as a parent, offering hands-on approach to learning and supportive mentoring as a peer. Advocate, refer and act as a liaison for families. Work closely with Health Care Professionals and work with the Public Health Nursing & Health Team to provide and deliver programs and services.

Duties :

• In-home support, utilizing a strength-based model with families who have children between the ages of 0-6 years;
• Teach individuals and groups about health promotion topics;
• Assist the Community Health Nurse to provide and deliver programs and services;
• Advocate, refer and act as a liaison for families;
• Work closely with other Health Care Professionals;
• Plan and deliver workshops and community education events;
• Develop appropriate awareness materials; administer the nutrition coupon program;
